I am getting ready to apply for a adjustment of status for my wife and I just want to make sure that I am correct about this.  We have to get another medical examination here in the United States from a certified doctor choosen by the government as well as  get all the vaccinations (She does not have any proof from the ones that she got in Russia) and all this informaion has to be sealed in an envelope.  She just had the medical exam in Moscow and now we have to do it again?

If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon

When we filed AOS we were told a copy of her shot record is all we needed. This was in late August of last year. When we went in for our interview (Dec 04) we were told she does have to go to a civil surgeon to have them check what shots she may need. They have to fill out a form stating she has all the shots. My wife did not have all the shots she needed but still only needed to get 3. They gave her the shots and sealed the form with a stamp and taped it up. For us we had to send it back in since they told us this at the interview. Once they received this my wife got her green card within a month.

You do not need another physical as long as it was within a year but you do need the sealed shot record. At least we did in San Diego.

Also when you go in make sure they know you only need the shot record. They tried to tell my wife she needed a full physical but since she came over on a K1 that is not the case. I had to tell the nurse many times that she only needs the shot record.

I guess you already found the part where she will not need another medical exam within a year. After filing, if they send her a standard form letter that says she needs to get a medical exam, just ignore it.

She should have received a copy of the Vaccination Supplement at her medical exam. If she has it, you can include a photocopy in the AOS application. Did she get any vaccinations during the medical? They were optional at that time. If she didn’t get any vaccinations or didn’t receive a copy of the supplement, then her file should have all the information the CIS will need to process her case. You should check with your local CIS office for any special filing instructions first. If they allow her to file in person, that would be best.

The shots required are different for each individual case, depending on various factors determined by a physician. I would not go to a civil surgeon yet or get any shots until the CIS instructs her on what to do.

Unless things have changed in the last 3 years your wife will need only the vaccinations. The Aos interview letter will let you know which ones your wife will need. I do not know why the INS does not post such shot requirements on their web site...at least I never saw it...
 A civil surgeon was a little tough for us to find, as it turned out our county health department had everything my wife and step son needed....at a very reasonable price.
