With all the current changes taking place with regards to the Fiancee interview process I cannot answer the questions asked of me by an individual who is becoming very concerened about his fiancee's upcoming intervew.

Can anyone answer the following questions?

From all I have been able to glean from the web sites, the forms the
fiance must take with her to her interview-

OF- 156 and  OF-156K

Have been replaced by the forms-

DS-156 and DS-156K

Is this correct?  At least the "DS" forms are the only ones you can
download from the Warsaw Embassy site!

Also, I cannot see anywhere on the Warsaw Embassy site anything about
needing to include the DS-230 part 2.  I know you have to send in Part 1
but I need to know for sure that you don't need to send in part 2!  Is
this
correct?

Also, it mentions on the Polish Embassy site very briefly about photos
with
the above forms needing to be full front shots (like a passport photo)
rather than an ADIT photo where they are turned a little to the side...
Is
this correct or can they be ADIT photos?

Can any of you guys recently going thru this procedure comment on the above questions?

"Form DS-230 Part I and II (Application for Immigrant Visa and Alien Registration) or DS-156 form (2 copies) and DS-156 Supplement (for fiancé(e) visas)."

Also, don't forget, the fee has gone up to $100!

"The fee for an immigrant visa is $335 (immigrant visa application + immigrant visa issuance); the fee for a fiancé(e) visa application is $100. Each applicant is charged in U.S. dollars or the equivalent in Polish zlotys according to the current Embassy exchange rate. The cashier accepts only undamaged, clean notes (without stains, inscriptions, stamps)".

Seems pretty clear...
hope this helps.

When we find the copies she made, we can answer with more certainty. After a while, all of the various forms sort of blur together...

But, I thought she filled out the OF-156. I asked her, and she remembered that she filled out the 156, but wasn't sure whether it was OF or DS (is there a difference???)

Whichever... she filled it out from the forms we downloaded from the embassy website, if that helps.

As for the photos, they actually use the photo for the picture on the Visa they issue (a stamp / label thing they paste in the passport), which is why they want the passport style.

We had something like twenty different photos for them to choose from. They were cheap to make, and I figured better safe than sorry, so we had a bunch taken from various abgles... with and without makeup... almost every passport / ADIT variant you can name... I don't know if they would have accepted the standard ADIT photo, but they clearly preferred the straight-on passport style photo.
