I am looking into going to Ukraine around a month from now,  and would like to know if I should send an envelope( with a return, stamped envelope) (enough time ??) to the Embassy, and just fill it out myself ?  or is there a realiable  Visa service on the internet that anyone might recomend ?

You should have plenty of time to get the visa through the Embassy.  You should write and request an application and send it via FedEx with a return FedEx.  That will take about a week.

My visa took the one week to get the original form that is required and then an additional 4 weeks for consulate to proocess the appication.  Given that I planned my trip in 6 weeks, I was wondering if I would have my passport and visa back in time to leave for the airport. Needless to say, I worry too much.

I have had great service from the Embassy - I have heard from others about somewhat slower service at the consulates in Chicago and NY.

I used the consulate in Chicago.  The visa I applied for had a stated processing time of 3 weeks.  Given that the consulate may have received a surge of applications at that particular time, four weeks was not so bad.  I knew I was on a tight schedule in the first place.

The moral of my previous post was supposed to be don't waste time if you are on a tight schedule.

Just appy to the embassy nearest to you.  You will need to send in your passport by courier.  If you are unable to visit embassy yourself then ask a travel agent that specialises in Ukraine/Russia to assist.
