Need good LINK for Packet 3 Checklist-

I have been to the Warsaw Consulate site but can't seem to find just the packet 3 checklist that supposedly you can download.  It is supposed to be a simple checklist that your girl is supposed to sign and send back..  Either I can't find it or can't download the one there, I don't know..

Any help appreciated..

It's not just a check list. There are a bunch of forms both in Russian and English.

The checklist form is just what she sends back to the Embassy to let them know that she is ready. But she needs the other forms, as well (I think)

It's what we're doing, anyway...

If that doesn't work let me know. I can email you the forms, I downloaded them
