K 1 visa

If the woman decides to return home before the 90 days and her baby ( I am the father, is a US citizen, does she need my permission to leave the country with our son.

Check with the airlines for their policy on this. They are about the only ones who would stop her and her child from boarding a flight out of the country without the father's permission.

Who has the child's passport?

Where was the child born? What jurisdiction are you in? Is the child holding a US passport? There is not enough information here to give you a complete answer. . . .

Child was born in Honduras, I live in San Diego, child has US passport.

What I see here is the need for custody orders. If the child entered the United States with her, then the child will probably be able to leave with her again without too much difficulty unless there are court orders to the contrary.

You should probably seek out a family law attorney in your area for an immediate consultation. . . . and for immediate filing of a paternity complaint to establish your parentage. . .
